Your informed consent means you:
- Have read and understood the information provided in this application
form about how your personal information and any police information
relevant to you will be handled and disclosed
- Provide your permission for the AuthNTick Identity Services to request
a Visa Entitlement Verification Online system (VEVO) on your behalf
- Provide your permission for the AuthNTick Identity Services to
disclose your information to any organisation listed in "Authorisation
for disclosure of personal information" of this form
- Acknowledge that I/the applicant understand the information on this
form.
- Acknowledge that the accredited AuthNTick Identity Services is
collecting information in this form to provide to the Home Affairs
department, for a VEVO to be conducted for the purpose outlined in
this form.
- Have fully and accurately completed this form, and the personal
information I/the applicant, have provided relates to me/the
applicant, and contains the full name and all names previously used by
me/the applicant.
- Acknowledge that withholding and/or providing misleading or false
information on this form and in any supplied identity documents is a
Commonwealth offence and may lead to prosecution under the Criminal
Code Act 1995 (Cth).
- Acknowledge that personal information that I/the applicant provided in
this form and on the supplied identity documents may be disclosed to
the accredited body named in Section C of this form (including
contractors, legal entity customers, related bodies or third parties
named in Personal Detail of this form in Australia or overseas, if
applicable).
- Acknowledge that any information sent by mail or electronically, in
relation to this form, including identity documents, is sent at my/the
applicant's risk and I/the applicant, am aware of the consequences of
sending information in these ways.
- Acknowledge that I/the applicant am aware that I/the applicant am
providing consent for a VEVO check to be conducted on all personal
information provided in this form and provided in supplied identity
documents.
- Give consent to:
- The Home Affairs disclosing my/the applicant's personal
information that I/the applicant, have provided in this form and
personal information contained in my supplied identity documents
to conduct a VEVO Check.
- Acknowledge that it is usual practice for my/the applicant's personal
information and police information to be used by police agencies and
the ACIC for law enforcement, including purposes set out in the
Australian Crime Commission Act 2002 (Cth).
Permitted Purpose
'Permitted Purpose' means the purpose for which the User Account was
issued, which is:
- In the case of employers, labour suppliers, industry bodies and
background screening or economy sharing companies, the purpose of
determining a person's entitlement to work in Australia;
- In the case of educational institutions, the following purposes:
- determining a person's eligibility to study in Australia; and
- informing a person who is enrolled at the institution of his or
her entitlement to work in Australia
- In the case of bodies issuing licences, permits or identity cards
which allow the holder to perform some kind of work, the purpose of
determining a person's suitability to hold the relevant licence,
permit or identification card;
- In the case of government agencies providing services to the public,
the following purpose of determining a person's:
- residence status where this is necessary to assess the person's
eligibility for services provided by that agency; or
- immigration status where this is necessary to assess the person's
eligibility for services provided by that agency;
- In the case of registered migration agents, the following purposes:
- determining a person's immigration status to assist in the
provision of immigration assistance; and
- informing a person of his or her right to work in Australia;
- In the case of financial institutions, the purpose of:
- determining a person's residence status in order to assess the
person's eligibility to open new accounts or for finance; and
- to verify the identity, residence status and lawful presence of a
person for the purposes of complying with the Anti-Money
Laundering and Counter-Terrorism Financing Act 2006 (Cth);
- In the case of officers of Medicare Australia, the purpose of
assisting in determining a person's eligibility for Medicare.
Disclaimer
- The Commonwealth provides the VEVO Website, and all Services, on an
'as is' and 'as available' basis for your use.
- The Commonwealth will endeavour to ensure that information provided
through the VEVO Website is accurate, but:
- makes no statements, representations or (to the full extent
permitted by law) warranties that the content of the VEVO Website
and information accessed through the VEVO Website is, or will be
at any time throughout the term of this Agreement, accurate,
complete, reliable, correct or free from error or omissions; and
- makes no representations about the suitability of information
provided on the VEVO Website for any particular purpose.
You should make your own judgements about those matters and should seek
independent advice before taking any action based on information accessed
via the VEVO Website. - If you rely upon information accessed via the VEVO Website that is
incorrect, and such reliance results in you being in breach of
Australian legislation, to the extent permitted by law, the
Commonwealth will take into account your reliance on such information
in determining whether the matter will be referred to the Director of
Public Prosecutions for prosecution, and deciding whether any other
action should be taken.
- To the extent permitted by law, the Commonwealth does not warrant
that:
- the VEVO Website will be available at any particular time or
location;
- the VEVO Website service, and access to it, will be uninterrupted;
- any defects or errors will be corrected; or
- the VEVO Website and its content are free from viruses or other
harmful components.
- As a result of hacking or other behaviour that is outside the control
of the Commonwealth, information or materials that may be offensive,
pornographic, unsuitable for minors' access or otherwise of a criminal
or violent nature may be accessible through the VEVO Website. The
Commonwealth makes no representations or warranties as to the
suitability of the information accessible for viewing through the VEVO
Website.
- You assume all risks associated with the use of the VEVO Website,
including (without limitation):
- The risk that the content of the VEVO Website and linked websites
complies with the laws of any country outside Australia.
